No, it wasn't the ballast, it was the LED Module. I purchased the entire headlight assembly off eBay (non Ford Part) and the LED module from a ford dealer. After replacing the LED module, they had to be synced using FORScan's Body Control Module headlight calibration.
Turns out it was a failed LED module, due to a cracked headlight housing, which allowed water to enter the headlight.
NOTE: When replacing headlight LED modules, there are many different versions (letter suffix on the PN) that must match the vehicle. I bough one off eBay from a a Ford Dealer...
Got into a little fender bender and cracked the headlight outter housing. Water got inside, and the headlight started going wonky. I replaced the headlight assembly, but transferred the two electrical modules with the old ones. New headlight doesn't work.
