4R70W, Direct Drum Question

So I recently lost 3-4 gear in my 2004 5.4L EB. Pulled the trans (4R70W) and disassembled. This is the first time I've done any transmission work, but everything that I inspected seemed to be fine. Except... the direct clutch drum. I read quite a few blog posts and watched some videos and was expecting to find the drum broken along a weld. Instead, I found the splines on the inside of the drum shaft completely stripped out. The part that mates with these splines I believe is referred to as the intermediate shaft or "stub shaft". I don't have another part for comparison, but the splines on this intermediate shaft appear to be fine. Also, all of the clutches that I inspected looked fine... nothing burnt up or locked together. So my question to those with experience is this... What would cause this type of damage? Obviously, I'm going to replace the direct drum along with clutches, seals and bearings... and... oh yeah, oil cooler and torque converter now since I assume that's where the stripped metal went. There was nothing in the pan :( But as to the cause of the direct drum damage, what else should I take a look at to avoid a repeat failure?

EDIT: I just looked at a diagram. Apparently the shaft that I mentioned above is called the "Direct Drive Shaft".
