When I shift from park to reverse or drive occasionally it will not move. If I rev the the engine to around 3500 rpm it will finally start to barely move and eventually start driving right.

I also get a long pause before engagement when shifting from D to R or vice versa. Doesn't seem to happen all the time, though.

You likely have an issue that should be checked. It will not get better, only worse and potentially more expensive.
