2000 expy does not have center axle disconnect if that is what your asking nor IWE's. They also come with open differentials. No locker or trac loc unit. According to my ford workshop manual you remove front axle assembly to replace the broken axle tube.

You may try cleaning the IAC valve. Heavier load with closed throttle puts different demands on it (more airflow required). It also controls minimum airflow for start up. Would check the throttle body as well for build up around the throttle plate when she is shut.
A lightning swap should be a direct bolt in as far as fit. Same chassis as an expedition. The question would be the pcm and wiring harness. Not sure of differences between 4v and 2v there. Fuel pump would need changed to higher output unit.

Manually shifting a 4R100 is not recommended. It's much harder on the tranny due to the way the shift is completed vs the pcm commanding it.
I do not know all the details but even highly modded lightnings running beefed up 4R100's recommend letting it shift itself. You can program the shift...
In the MD/DC area they salt the roads heavily. Severe rust on the undercarriage of mine. What's interesting is the frame is fine. The thin sheet metal has not done so well.

Make sure you check thoroughly under the body for rust. Particularly if its 4wd and if they salt the roads in your area?! Many things can be fixed but rust is hard to deal with.

My guess if an exhaust port truly got completely blocked would be valve train issue on that cylinder that impeded exhaust flow or burning high quantities of oil.
I would do a compression test as well as a leak down test to check the condition of each cylinder.
I would say there is something wrong with the cylinder(s) where the exhaust ports were blocked?! I don't see how that is possible if the cylinders were firing normally. A few misfires should not block the exhaust manifold. Cats maybe if they overheat from raw fuel.

These are the type extensions I like to use. There are other manufacturers of similar type that lock onto the socket. Its a PITA when the extensions pull out in situations liking changing the #4 spark plug and can be a biotch. These are great for avoiding that....

I do #4 by feel now with the correct assortment of extensions and universal joint. Gave on seeing what I was doing long ago. Locking type extensions come in real handy.
