    You may try cleaning the IAC valve. Heavier load with closed throttle puts different demands on it (more airflow required). It also controls minimum airflow for start up. Would check the throttle body as well for build up around the throttle plate when she is shut.

    Good luck! Make sure you check thoroughly under the body for rust. Particularly if its 4wd and if they salt the roads in your area?! Many things can be fixed but rust is hard to deal with.

    My guess if an exhaust port truly got completely blocked would be valve train issue on that cylinder that impeded exhaust flow or burning high quantities of oil. I would do a compression test as well as a leak down test to check the condition of each cylinder.
