Title says it all..
Came with owners manual but no card with door code. Any help?
Check the inside of the interior fuse panel cover. It is on the right side of the passenger side footwell.
You have to remove the fuse panel on the passenger side, the code is on a label behind it...

The used car dealer was unable to find the code online. I used the YouTube video with the two keys. Worked.
I later found the code card in a sleeve with the owners manual.
I had this with an old Mercury Marquis is used to have. My mechanic hooked up and told me the code in a couple minutes for free. The Ford Dealer wanted $50 to give me the code. If you have a good mechanic, give him a call.
Wow! You have the owner's manual and you STILL couldn't find the code? Perhaps the manual is defective? Here's pages from mine. I hope this helps you out.
