Jerking

My 99 4.6L is having some problems
What I find is that of the initial start the truck will jerk quickly. If you cruise a slow speeds it'll either jerk or bump a lot. You can definately feel it. I find if I load it with a trailer it becomes more evident. I have EGR problems before which are now solved with a entirely new EGR system. I find if I put it into 4wd it goes away, sometimes depending on how the transfer case catches it (in a jerk or not) when it engages I might feel a big jolt, jerk, or thump. After that I ussually can't get it to come back while still in 4wd.

No codes so far, going to recheck it.

What is your synopsis?
I'm not well versed in transmissions, but it sounds like a gearing issue.
