The RCD suspension lift is the 'proper' way to do it. Don't waste your time and money on a spacer lift, if you have plans on a 'real' lift in the future.
A dirty IAC can cause this.
Also this can get more pronounced when the air conditioning (or defroster) is on, as the iac will bump up the idle to make up for the additional load on the motor.
A leak down test will help you determine id you have a leaky head gasket.
I would put a pressure tester on the radiator and see what the pressure really is when the truck is running.
If you want to stay 'old school' check this first:
On your little cassette adapter thing, make sure when you spin one of the wheels that the other one spins too. If it doesn't, then you need a new adapter. If the 'feed wheel' doesn't spin, it triggers the cassette player to reverse.
Leave them, you don't want those metal surfaces banging together. Remember, because of the angle, an inch or two at the bump stop means 4" at the wheel.
I'm 90% sure that you can't use newer style keys (remotes) on the older vehicles.

If you're really handy, have the tools and equipment, and can find a truck that is totaled from the rear as a donor, then it may be an option. Otherwise just buy a 4x4.

You can certainly do that. Eaton makes an e-locker, there are also selectable lockers from ARB and others, and even cable actuated lockers. They aren't cheap, but they will improve the performance of your 2wd.
For one of my projects, I just went to a truck stop / truck store and picked up some plain, black trailer mud flaps and cut them to fit the way that I wanted them to.
It's actually urethane. (Neoprene is what they make wet suits from ) They eliminate a lot of the slop of the stock rubber bushings. If you replace everything with urethane, you will notice a positive difference. It also lasts much longer than the stock rubber.
They can squeak if not installed...
I would say that you probably need to get under it and measure the length of that idler arm. If you are going to change out the steering components, you may as well do that idler arm too.
It sounds like Integrated Wheel Ends. When they don't get enough vacuum or start leaking, they will engage. The vacuum keeps them 'unlocked'.
I've always felt that Ford designed these backwards, since most people have their truck in 2wd 90% of the time.
The Integrated Wheel Ends will only disengage when the engine is running and 4wd is engaged. I can't remember the exact setup that they used in '99.
My '98 had 4hi, awd and used a center axle disconnect system. I though that they eliminated the CAD and went to IWE's in '99.

Tires can cause this, but I don't think it's the case here. Mainly because your rear tires are turning (slightly) slower than the fronts. The system wouldn't detect slipping, which is rears turning faster than the fronts.

The system would have to be set up for forced induction. The fuel curve and perhaps the injectors would have to be adjusted to make sure that it doesn't run lean.
