As SafariGoneWrong noted this is the part number I used on my 2004 as well 4L3Z-3A788-AA. I think the lengths may be slightly different for the Expedition and F-150, but it'll fit just fine.

I sure don't recall seeing any smoke from the EGR. There's a rubber diaphragm in the top there that should seal off the top half from the bottom half I believe. So I don't think smoke should be getting to that hole. Unless I'm wrong the vacuum line on top of the EGR will be able to suck...

I agree it appears you could simply connection hose 1 to hose 5 and bypass both heater cores. This may very well be the single most helpful part of your diagram as people need to bypass temporarily.
