I may have posted this earlier today, don't see it now. I have been having a lot of problems with the fail safe mode. Started about 4 months ago, I went to premium gas and a lot of additive cleaner. It stopped and then started again last week. I have been using premium fuel. Any thoughts? I know it has shown up on posts and there have been a lot of complaints. Suggestions

I had it happen about a month ago. From what I have read, it is related to the throttle body. I do believe the TPS is built-in to the 3V throttle bodies so you have to replace the whole thing. I'm not saying yours is definitely that, but I have read it happen on F150's.

The only time that ever happened to me was due to overtemp condition, caused by low coolant level, caused by blow off from the coolant bottle, caused by a failed head gasket, which was allowing compression/exhaust gas to enter, and overpressurize cooling system. (I could smell exhaust in my bottle) Fixing the head gasket remedied this- NOT saying this is your problem, but you may want to check your coolant level, and check for any leaks.
