I have 2018 Exp Limited with just over 100K. Bought used. I occasionally notice surging during acceleration through 4th and 5th gears. Is this an indication of issues with the trans? Once it shifts up to 6 and higher, I don't seem to have that issue.

Maybe try and get the PCM programmed so that it goes through the gears one at a time and doesn't skip around. Pretty much all the transmissions from 2018 to December 2022/January of 2023 are going to have CDF drum failure. Starting 2023 they put the revised CDF unit in. Just drive it. Mine does the same thing, upshifts, downshifts and clunks but still going at 130K.

There's a Technical Service Bulletin / Recall that may fix this. I just had it and the emissions update performed. The one for the drive train is for erratic shifting among other things. The one for the emissions is for the catalytic converter warning code going off prematurely, making one thing the catalytic converter is bad.
