Can anyone identify this noise?

Been trying to resolve a rough idle/rich state on my 2000 Exp. 5.4L.

Of course, I've opted to go with the blind replacement of common problems: COPs, plugs, cleaned IAC and PCV.
Replacing/cleaning these has helped, but not solved the issue.

I'm awaiting the arrival of my OBDII scanner so I can get codes. Haven't wanted to drive it because it has an awful time idling and it chokes down; barely holds a 500PRM idle.

That all said, I'm getting this god awful racket coming from the passenger side. The link is to video. You can hear this even with the hood closed, so it's rather loud. Curious if anyone can identify it.

My first thought is timing chain guide or tensioner failure on the passenger side. Chain may be eating into timing cover or guide and as a result valve timing can be all over the place.

Next thought would be very bad bearing on one of the accessories or tensioner pulley. That would be easy to confirm by removing the accessory belt when its cold and run it. (Very short time only!! No alt or waterpump functioning)

But I would suggest you get a mechanic stethoscope and narrow it down.

Noise

Just like toms89 stated I would pull the belt and check for the noise.
I had a very similar rattling noise recently at startup, but it would go away after reaching operating temps.

Since I had 175k I just went ahead and performed a complete coolant replacement.

New water pump
New thermostat
New fan clutch
New hoses, belts and a flush and fill.

It was most likely my water pump because the noise is gone now.

agree on above; pulley/tensioner on main or accessory unit.
a/c units are also prone to this noise too if not used in a while.
