Why does my coolant reservoir look like this?
Any advice on what should be done? Thanks.

Blown head gasket ? How many miles on the motor ? That return hose looks swelled up in the picture, but how does it look in person? Is it soft-ish ? Shouldn't be soft. Has your reservoir been blowing out any coolant ? Any overheating ? Does the coolant smell like exhaust gases ? When's the last time the coolant was changed ?

Hope not. It drives perfectly never gets hot. How many miles on the motor?

Yeah, it shouldn't have a blown head gasket at that mileage. But you have to change the coolant every few years to help prevent block corrosion and scale buildup in the system. If it's the original thermostat it might be worth pulling that out and replacing it too.

You can use a bottle brush to clean some of that residue out of the reservoir tank, or if you just want to replace it they're still available for about $50 .

Same as mine here! Since the unit is old enough, better flush it first tap water with engine running. Remove the return hose connection on the upper part of the radiator. Rinse it until the water coming out will be clear. After flushing the system check the thermostat sensor and the water pump fins if it is rusted or deteriorated already. Then, fill-in new coolant. I found out that my water pump fins has gone due to rusting problem and i replaced it with a new one. I hope this will help.
