It wasn't hard to replace it took less than 5 minutes and was about 25 bucks. The old one just pulls out of the air intake tube and push the new one in

I have an 01 5.4lt in Phialadelphia with the same issues, I'm ready to drive it through a wall. I've cleaned my MAF and throttle body a few times and it always comes back on after about 60 miles.

PCV rotted tube

Now, I am hearing by a lot of people that its the PCV tube. If you search google people say its a Ford part number and its the elbow BEHIND the throttle body that is very hard to see. I guess it dry rots and that is the main cause for these codes. It is a B**CH to get to that's why I haven't replaced mine yet. I don't know how people change it but I will eventually do it once it gets warm here in NH.
