2000 EXPY doesn't want to stay running when it's cold

In south central PA it's been freaking cold. A couple weeks ago it was -30 with wind chill. This week we've had a few days of 2 degrees or -2 degrees and in the mornings when it's that cold, my truck will start, idle at about 600rpm for MAYBE 2 seconds, and shut off. I don't think it's battery related (I know usually cold starting problems are) because it fires right up, it just shuts right off. Takes 3 or 4 times to get it to stay running.

Not garage kept.

So what is wrong with my truck? Any thoughts?

how many miles on the expy? when was the last time plugs wires etc. were changed When was the last time the throttle body exhaust ports were cleaned?
Check/clean IAC Valve
