Heater-knob not changing where it comes out
Yes the air flow (defroster, vent, floor) is vacuum controlled. The Temp (hot/cold) is controlled electronically (that's the blend door). Check your vacuum lines and reservoir. With out vacuum it defaults to defroster- driveway2fast

99 Expy No Vent Air?
Probably still the blend door (although still check the actuator). What happens with the blend door, is the top of blend door hinge mates with the actuator and being a plastic part subject to dramatic changes in temperature becomes brittle and breaks. Blend door issue?
Most likely NOT the blend door (that is controlled electronically) Climate control system uses vacuum to direct the air flow (vent, floor, defrost etc..) so A vacuum leak would be different symptoms As mentioned above I would check your coolant level first, then water flow/thermostat- driveway2fast
