I can't answer some of your questions but I can on the factory code. I'm not sure about 1st Gen Expeditions but on the 2nd the factory door code is located in both the manual in the glove box and on the system itself which on the 2nd gen is the outside kick panel on the passenger side, by the fuses. I hope this helps.
