Have a 2009 with a Fan Switch (AC/Heater Fan) which only works on 1, 2, and 4. I have replaced the Fan Resistor Board and it didn't change anything therefore I am thinking I must have a bad switch. I can't find anything on line about replacing it (or a good part number to order one). Anyone have any experience with replacing one of these?

If you have a meter you can check the switch before purchasing a replacement. Also you could check for voltage at the resistor pack for that speed.

Any links on removing the Dash Plate to get to it? It seems I remember seeing one a long time ago, but I can't remember where.

Don't know about the 09's but the earlier versions either snapped on and/or bolted at the bottom. Try prying / twisting with a wide plastic putty knife. Just remember if your prying and you think it is going to break - it will.
