My two cents - if I may... I had codes for both bank1 and bank 2 too lean. Had the fuel filter changed - codes have been gone for over 5K miles.
No clue as to the rest of the problems.

We'll you got lucky Bruce on that one. Usually when a fuel filter clogs up, it will affect more than just 1 bank lean condition, due to the fact it's starving all the injectors. If shops and customers would just change the filter every30k or so, you'd never have to guess, or hook up a fuel pressure gauge to test. But once again regular scheduled maintenance seems to elude most!
