5.4 nightmare

Hi guys I’m having issues diagnosing this 5.4 2003. I’ve installed a new intake manifold due to being cracked,new Pcv hoses, new spark plugs,cleaned iac valve,cleaned fuel filter,new injector o-rings. It still has a rough idle when in drive or reverse. Fault codes are P2195 & P0135. I performed a power balance and these were my results.BD84AF3C-D2CA-4690-B809-BA2D233E69ED.jpeg

Not easy them engines! One solace is Makuloco.. He does amazing job at diagnostics and also sharing his input and repair methods. Il start with the below:
Start watching his videos.. I rtemember one that shows similar issue as you report but too many to go through. Hope you find the one the helps you out.

Looks like cylinder 3 is giving you a miss ? Did you say you cleaned the MAF ? Move the coil from cylinder 3 to another cylinder and see if the big dip follows. Or if you have an extra coil laying around put the new one on #3 and see if the dip goes away. Clear the codes and see if anything comes back. Do you have a code reader that will read live data ? ( Like a Foxwell 301 or something...). You'll have to look at the forward oxygen sensors and see which one is lame ( the P0135 code...).

@joseaguirre Make sure that all injector and coil connectors are solidly connected. I had the exact same problem as you right after I replaced my intake, and found that one of the injector connectors had backed out, which is an easy fix. A few of my coil and injector connectors have broken clasps from being old and brittle. And of course, double check for vacuum leaks and make sure you reconnected all the vacuum hoses correctly, as it is easy to lose track of those sometimes.

After trying the thing mentioned by Ham and Vega, try swapping the Cylinder 3 plug, coil and injector with another cylinder and see if the problem follows them. If it doesn't, do a compression test of the cylinder.
