Torque converter issue?

We have a small issue and we're not sure how serious it is. When my wife is driving her Expy, it seems like the transmission is having a little trouble. Somewhere in the 40-60mph range (normal acceleration, driving around town), there is a little hesitation -- not quite a bucking. The truck seems to "struggle" a bit. My father-in-law suggested it could be the torque converter. When I turn the O/D off, the issue seems to go away.

No CEL, but I need to hook up Torque to see if anything is pending. A couple months ago, I was able to drain/replace half the ATF (the issue prompted me to attempt this as a solution). I don't think it's a misfire as I changed the plugs a few months ago.

Thoughts?

I associate hesitation with the engine. Now it could be when it goes into overdrive as that may put the greatest load on the motor. (Overdrive @ low rpm) Turning overdrive off may simply be reducing the load eliminating the symptom.

Now engine flare or slow to shift I would associate with the tranny or possibly tune / sensor.
