It is on my list of things to do. I knew a guy that had the setup on a newer F150 - definitely would scare the crap out of you and get your attention.

If your going to do it I'd also suggest an air tank w/drain. Should be able to source them from a trucking junk yard. Most of the larger (real) auto parts stores also sell big truck parts including the nylon air lines, fittings etc.

I like the ones that sound like the old steam engines...

They have some that are all self contained and others you need to piece together, all depends on what you want to spend

Here are mine, Wolo 847 "Siberian Express":

They're not actual metal train horns, but plastic clones. But they're big as hell (the longest of the three is 2 ft long) and they're loud and scare the shit out of you. I was too cheap to buy an on-board compressor and instead bought a 5 gallon air tank with a schrader intake valve. I'm filling it from my garage compressor. A full tank last about 20 horn blasts, which is more than I do in a week.

I've installed the horns under the bumper behind the rear wheels. Had to separate them from their manifold in order to make them fit. The tank is in the rear cargo area.

The whole deal cost me about $140: $80 for the horns and valve (amazon), $40 for the tank (Walmart) and then some for cables, horn button, etc.

A real set of actual train horns runs into the hundreds of $, plus about $300-400 for the air system.

I want a set so bad. There's a dude on another forum I'm a member of that works for the railroad and had a garage full of k5la's that he was trying to sell or trade for parts to bag his truck. I haven't been on there in a while, I might have to check in there and see if he's still got some.
