Is it worth the pain and hassle of installing headers or would you guys leave the stock exhaust manifold alone? If so, what's available out there?

I am in the midst of installing a set of Gibson headers on my 4.6 right now. The fit seems to be good, albeit a bit tight but other than one broken stud inside the head and one that was snapped off leaving about an inch of thread showing no major issues.

when i bought my X it already had an exhaust leak so for me this was a two part solution, one part fix, one part enhancement.

i'll update when i am finished with the install.

I had Gibson headers installed on my 99 4.6L. I also installed them with a gibson catback swept side exhaust. They added a lot of acceleration to it. I had professionally installed which from what I've seen after looking under it requires the engine to be shifted slightly to get them in. Mind you the whole setup cost me almost 2000 to get on the truck so I'd expect them to work well.

I had a lug blow out on the driver side manifold, so mine was also a two part solution.

I'd only regret it if I get rid of the truck to get a 7.3L excursion because 4.6L is ******** to begin with. I needed it for towing. Otherwise it gives the truck some much needed balls.

It'd be more worth it to do it on a 5.4, maybe wait until your next truck or take this opportunity to sell it.
